Case Management

Case management is a collaborative process in which our family advocates work with guest families to provide the tools and resources necessary to attain self sufficiency.

Case management is the process in which our trained Family Advocates collaborate with guest families to determine the needs and goals of the family, and institute action to obtain those goals and meet guest expectations. Our Family Advocates are responsible for helping guest families evaluate and understand all the options available to them and, when appropriate, help guests coordinate with other agencies to meet a specific need.

Case management is at the core of Family Promise of Hamilton County by providing our families with the necessary tools and resources needed for families to achieve success and sustain it.

Homelessness is not caused by one issue alone and Supportive Services help by connecting guests to multiple resources to help address all of the underlying causes to a family's unique situation.
